Trusted Partner Practitioner

Salary £90 per completed review

Covering the West Suffolk area

Flexible Hours – Monday to Friday

Empanda is working in partnership with Suffolk County Council Adult and Community Services as part of the Trusted Partner Organisation (TPO) contract, reviewing care and support plans for local people in receipt of care and support services and we have an exciting opportunity for an enthusiastic and passionate individual to be part of an innovative social care review team.

About the role

The role of trusted partner practitioner is home-based. You will be working with customers living in West Suffolk and your role would involve meeting individuals in their own home to review their care and support plan (assessments can also be carried out over the phone depending on customer choice), to identify whether it continues to meet their needs and to write up new care and support plans using Suffolk County Council’s customer database. It is expected that you will complete a minimum of three reviews per week.

You will have experience of working in a care and support environment, preferably within services for older people and/or adults with a learning disability. An understanding of person-centred, outcome focused care and support planning, risk assessments, safeguarding and MCA/DoLS would be advantageous.

The ability to work alone and unsupervised, managing a caseload and meeting deadlines, is essential.
